MTI Forms
The MTI scheme application and other forms can be completed online by clicking the relevant form at the bottom of the page.
Which form do I need?
MTI Application form: for NHS employing organisations and royal colleges who wish to apply to the Academy for a Temporary Work – Government Authorised Exchange Certificate of Sponsorship (CoS).
MTI Extension form: for NHS employing organisations who wish to extend an MTI placement. A placement can only be extended if the initial visa was issued for less than 24 months. International doctors on the MTI scheme cannot stay in the UK longer than 24 months.
MTI Transfer form: for NHS employing organisations when a doctor on the MTI scheme requests to transfer to another NHS organisation during their placement.
Deanery approval form: for the local deanery to approve MTI posts.
The only people authorised to sign the Deanery approval form are on this list of MTI authorised Deanery signatories.
Data Protection Consent form: for the MTI doctor to sign and send to their NHS employing organisation who will submit the form to the Academy with the doctors application or report.
MTI starters report: for NHS employing organisations to notify the Academy that a doctor has started their post.
A verified PDF copy of the doctor’s Right to Work profile must be sent within ten working days of the doctor’s first day of employment.
MTI exception report: for NHS employing organisations to inform the Academy of any changes in the doctor’s circumstances or if a doctor fails to start work within 28 days of the agreed start date listed on their CoS.
MTI end of placement report: for NHS employing organisations to inform the Academy that an MTI placement has come to an end.
